Accountability becomes credible only after leadership can see the conditions producing the outcome.
Increasing targets, reporting, or pressure may improve performance when the issue is effort or discipline. But when leaders cannot see decision delays, capacity constraints, handoff failures, policy burden, technology limitations, or conflicting measures, accountability can push people to work harder around the system instead of improving it.
